Continuing Care
After your initial visit, most of the value is in what happens between visits: a dose that needs adjusting, a refill, a lab result to read. Continuing Care covers that, on a monthly basis, so you are not booking a visit every time.
Membership is optional. The initial consultation does not change, and paying per visit stays available: follow-up visits are $100 for 30 minutes, booked as you need them. The membership changes how you reach the practice between visits, not the care you get at them.

Weight-Loss Track
For patients on GLP-1 medication

per month while your dose is being adjusted · $99 a month at maintenance
- A visit every month during titration, with side-effect management between visits

- Refills and lab orders managed between visits
- Moves to the $99 Continuing Care rate once your dose is stable
How to join
Choose an option above, enter your name and email, and send the request. Holly is notified and replies with a secure payment link; your membership starts the day you pay, and nothing is charged until then. Online checkout will be added later.
3-month minimum, then cancel anytime. Membership is billed monthly and covers clinician time and care coordination between visits. It is not insurance and not a prepaid plan for medications or labs.
After the minimum term your membership renews month to month until you cancel. Cancellation takes effect at the end of the current billing month; there are no partial-month refunds, and unused visits do not carry over.

Medications and lab work are always paid directly to the pharmacy and the lab. They are never bundled into the membership.
HSA/FSA: membership fees may be HSA/FSA-eligible; your plan administrator makes that determination. We provide an itemized receipt on request.
Membership is for established patients who have completed an initial consultation. Whether ongoing treatment is appropriate is a clinical decision made at your visits, not something the membership decides.
